How to Refer Patients to ADI :
1) Fill out an ADI Imaging Prescription Form (referral form).
2) Give completed ADI referral form to patient.
3) Fax a copy of the referral form to our office to ensure that we receive the information.

4) Phone our office to make an appointment for a 3-D scan.

5) Patient brings in referral form to our office on the day of appointment.
6) Scan of patient is completed in about 10 seconds generating 512 CT slice images. 3-D images are generated from this complete volume data.
7) Post-scan 3-D secondary reconstruction information is mailed back to referring clinician within a few days.
